Dear Tutors,

Welcome to our program and thank you for your time and energy as you change the lives of others with your generosity!

Because LV-DAC needs grants and contributions to survive, we must keep statistics to prove our success with Learners.

You are an important part of the statistic-gathering process. Be sure to turn in your Monthly Attendance Report(s) for your Learner(s) by the 4th day of the following month.  Click on the link below to fill out online or download.

The Tutor / Learner Agreement is to be read/completed/signed/dated by you and your Learner(s).  Keep a copy for yourselves and return the original.  Click on the link below to fill out online or download.  Please drop off the Attendance Reports and Agreements at the Reception Desk in the Quintana Learning Center, email them to lv-dac@dacc.nmsu.edu or mail them to:
